PROXY FORM

1. The AGM Notice you got by regular mail has a blank proxy form (last page). 
2. Or download and print the proxy form below. (Please scroll down) 
3. Fill it out with a dark ink pen.
4. Remember to sign. The property manager will verify the signature.  
5. Scan the completed form.
6. Email as an attachment to  manager.ave@quaypacific.com, mention Unit# and LMS3187 AGM Proxy in the subject line.  CC your proxy just in case.

Only Owners can assign a proxy. If your spouse, common-law partner, or family member is your proxy and they do not also own your strata lot, you must complete the proxy form so that they receive the voting card. 

If you don't have a printer, you can send a photo of the completed proxy form.

You could also give the completed proxy form to your proxy person, and the proxy can arrive at the AGM and present it to the property manager during the "Registration". Proxy assignments must be accepted right until the AGM starts ("called to order"). The deadline (5 pm on Wednesday, June 25, 2025) mentioned in the proxy form is a deadline of "convenience" for the property manager to verify the proxy form.  

As mentioned, the proxy assignment must be accepted till the AGM starts.  

You can assign anyone as your proxy. It does not have to be someone who lives or owns a Ave strata lot.   They must be anyone who will attend the AGM.

On AGM day, if you change your mind and can attend the AGM yourself, upon arrival prior or during registration, you can inform the property manager that you are the Owner and would like to vote instead of your proxy. The property manager will then (re)-assign the voting card to you before the AGM begins. 

In addition to the above persons, there could be others who are available to attend, and you can assign anyone. Please ensure your proxy will attend the AGM and arrive on time (Thursday, June 26, 2025, 6:00 pm) at the Amenity Room of the Ave.  If you assign someone who does not live in the Ave, please ensure they will be able to enter the building.
